Health Certificate for Residence Application
All new international students are required to finish Health Certificate for Residence Application in order to obtain an R.O.C. resident visa.

For information about licensed hospitals, please check with R.O.C. Embassy, Consulate or Mission in your country.

Please scan the first 2 pages of the verified Health Certificate for Residence Application and upload it to the following website MyFCU.

Student Health Examination
Another paid health examination will be needed after students register at Feng Chia University.

Time: March.5th, (Friday).During Physical Education course.

Place: Recreation Bldg.

Fee: NT550 / per person

Please follow the 5 steps to finish it.
(1) Visit the following Myfcu
(2) Key in User and Password.
(3) Click Registration (red one)
(4) Click "Fill in Health Self-Assessment Card" to fill the information and then Upload health check report.
